P.S. Are you new to downloading files?

Our New Patient form is a PDF file. When you click on a PDF link, your web browser — probably Internet Explorer — will open the form for you right on your screen, or else it will automatically download a copy to your desktop. (What happens depends on your browser’s settings.) If you are on a dial-up connection, retrieving the file may take a while.

If you don’t want the PDF file to automatically open on your screen and prefer to download it to your desktop, simply right-click with your mouse and select the appropriate option. That’s all there is to it!

NOTE: PDF files require Adobe Acrobat Reader, which is pre-installed on nearly every computer, no matter what operating system you use. If you don’t have Adobe Acrobat Reader already installed, you can get it by going to Adobe.
